Catherine Matheson [*] NAVAJO FAMILIES WHO HAVE CHILDREN WITH DISABILITIES In the United States, many have come to believe that families and family involvement are the most important factors in supporting children with disabilities. Families exist within an ecocultural environment that is complex and powerful, and it is within the family context that development in all its forms begins and is nurtured. We cannot understand what family involvement means without understanding the culture of families and the informal systems that families use to adapt and operate. In this chapter, the example of Navajo families with children with disabilities underscores and shows in high relief the heavy reliance on cultural support for families that is part of any family system. As early-intervention and family support programs continue to grow in the United States, we need to understand and sustain those benefits of indigenous systems that provide support and relief to families even as we seek to engage families in the use of supports that may be offered by private and public agencies. Two complementary systems operate to fulfill the needs of Navajo and other American families who have children with disabilities: one is the indigenous adaptive system and the other is the public health system that has been established nationally in the United States with local options. One example is the Individuals with Disabilities Education Act (IDEA 1997), parts B and C, providing early intervention, preschool, and school-age services. The indigenous system was in existence long before the IDEA, but has not been recognized as such. Both systems attempt to increase the well-being of families, but because all adaptive systems have a cost, neither system is totally benign. Each system extracts some payment for the benefits it provides. When discussing costs, our intent is not so much to point out financial costs, which are often involved, but to show that costs to families can be more subtle, such as the erosion of family responsibility and satisfaction with the efforts the family makes on behalf of the child with a disability. Thus, in the Navajo sample, families identified medical disabilities and sought help for medical problems from professionals but were sometimes unable to resolve their anxiety until they involved spiritual services or indigenous healing practices. Conversely, Navajo families did not identify learning disorders in the preschool age group that we interviewed. Learning and language disabilities were primarily identified by professionals and schools, who in turn sometimes found families apprehensive and reluctant to adopt an active role in early-intervention efforts that they had not sought themselves.

Depending upon who is telling the story, the Endangered Species Act (ESA) (1) is either the “pit bull of environmental laws” (2) or a political fig leaf providing cover for business as usual. (3) Those differing views are mirrored by opinions about the scientific underpinnings of the Act and its implementation. Environmentalists insist that political considerations lead the United States Fish Wildlife Service and NOAA Fisheries, the two federal agencies responsible for implementing the ESA, to ignore science in favor of development, while the regulated community sees precisely the opposite problem. (4) Both supporters and opponents of the ESA are intent on gaining the scientific high ground. In part, that desire undoubtedly stems from the Act itself, which requires that a number of key decisions rest upon the best available scientific information. (5) But it goes deeper than that. Scientific victory seems to hold the key to political victory. Both sides seem convinced that public opinion turns on whether, and to what extent, science supports the decisions of the agencies responsible for implementing the ESA. There is good reason to think that this assumption is accurate. As a society, we hunger for objective, rule-based decision making, especially when the decision pits human interests against those of another species. We worry that decisions lacking a firm, objective basis may be arbitrary, wholly “political,” wholly dependent upon the whims of the particular decision maker, or made on the basis of improper motivations. We look to “science” to provide the objectivity we crave. The demand for strong scientific support of ESA decisions will only grow in the future, as the costs of conservation measures become higher and more apparent. (6) Science, however, is not as objective or neutral a basis for decisions as we might hope. In recent years, the uncertainties and gaps in the supposedly “scientific” decision making of the ESA have become increasingly apparent, and increasingly the source of controversy and contention. (7) At the same time, several reviews have strongly endorsed the use of science in making decisions under the ESA. (8) That juxtaposition suggests that different observers are looking for different things from the use of science in these decisions. The controversies raise important questions about how science is actually being used, what we are seeking from science, what we are actually getting from it, and what we should realistically expect.

Some physicians who perform only tumescent liposuction question the benefits of and need for laser lipolysis. I acknowledge that we are still gaining experience with laser-assisted liposuction, and that there are many technologies, such as ultrasound, that have come and gone with liposuction to try to facilitate the procedure. But the improved cosmetic results that can be obtained with laser-assisted liposuction and its safety record so far seem to point to an expansion of its use. The crisscross pattern of cannula movement used in normal liposuction often leaves pockets of fat behind that cannot be removed, but a laser can liquify these areas. In addition, the skin overlying the areas of fat removal contracts because the laser coagulates the tissue, inducing collagen retraction and tightening of tissue. This effect is an added benefit that is especially useful for liposuction candidates who would be able to reduce their fat volume with normal liposuction but would not have accompanying skin contraction. The laser can break up fibrous tissue for scar subcision and be used without suction for the reduction of small areas of fat. We may also see a reduction in bruising and postoperative pain. Some studies have found that laser-assisted lipolysis and liposuction produced less bleeding than did liposuction alone. Histologic analysis of laser-assisted lipolysis using 1,064 nm have shown coagulated blood vessels and a reorganization of the reticular dermis and collagen without nerve damage. Scanning electron microscopy images have shown specific ruptured adipocytes.

ANDOVER, Mass. — Royal Philips Electronics today announced U.S. Food and Drug Administration (FDA) clearance of the HeartStart Home Defibrillator for sale without a prescription. The FDAs decision follows the favorable consensus of the Circulatory System Devices Panel, which met on July 29, 2004 and supported removing the prescription requirement. Defibrillators provide therapy for the most common cause of sudden cardiac arrest (SCA), a leading cause of death among adults in the U.S. The Philips HeartStart Home Defibrillator was the first new-generation defibrillator specifically designed for home use. It is now the first defibrillator cleared for over-the-counter sale, a move that will help make the HeartStart Home Defibrillator more easily available to consumers for home use, where the majority of cardiac arrests occur. “It is important to recognize that for a substantial percentage of SCA victims, cardiac arrest is the first sign of heart disease,” said Dr. Jeremy Ruskin, Director of the Cardiac Arrhythmia Service at Massachusetts General Hospital. “We know that access to early defibrillation helps save lives. Removal of the prescription requirement for the HeartStart Home Defibrillator gives people freedom of choice to be better prepared for unexpected cardiac arrest–a rare but life threatening emergency.” Sudden cardiac arrest is a leading cause of death in the United States, claiming approximately 340,000 lives each year. At the recent Circulatory System Devices panel meeting, physicians, professional organizations and cardiac arrest survivors concurred that greater access to defibrillators will help save lives from what is often a treatable, yet unpredictable, emergency. Cleared by the FDA in 2002 for sale by prescription, the HeartStart Home Defibrillator was the first new-generation defibrillator designed specifically for use in the home. The advertising and public relations world has continually brought physicians names to the public. Ten years ago if a doctor called himself a cosmetic surgeon, the patient knew precisely what that title meant. Currently, doctors in various sub-specialties, such as dermatologists, gynecologists, opthamologists and even dentists, are performing cosmetic surgery procedures. This can utterly confuse prospective patients trying to find the most qualified surgeons. How, wonders Dr. Stanley Taub, a New York City plastic and reconstructive surgeon, “is a prospective patient to know who is a diligent, excellent, caring physician and who is a charlatan?” Taub wants to disseminate information the public can use to select a cosmetic surgeon in a country now brimming with doctors calling themselves “cosmetic surgeons.” “Public law and policy do not necessarily protect us from unreasonable and untrue claims,” declares Taub. “A physician needs to be licensed in medicine which means earning a recognized medical degree–and can hang out a shingle in any specialty he chooses. The state does not control what area of medicine he practices or which type of specialist he says he is unless there are complaints and he is brought before the Office of Professional Medical Conduct. Often, however, by then it is too late.” How do we ascertain a physicians expertise and qualifications before placing ourselves under his or her care? Four basic areas must be investigated–a physicians training, certification, associations and attitude toward his or her patients. “A physician should have and be willing to provide evidence that he or she has trained in the area of their expertise,” Taub states. “For example, patients want to locate a plastic surgeon who trained in plastic surgery, which means a post-surgical residency fellowship in the sub-specialty. Each residency or fellowship program gives a certificate [indicating] the dates of service as well as the hospital or department in which the physician trained.” Most physicians keep certificates and other diplomas framed in their offices. If these are unavailable, at least a curriculum vitae or brochure describing a doctors training should be available and carefully examined by the patient. “If a physician is either unable or unwilling to disclose this information, it becomes a clear issue of medical consumer beware,” Taub warns.

The laser industry is poised for substantial growth, reports Frost Sullivan (Mountain View, CA). Global demand for industrial laser systems should grow from $1.2 billion in 1992 to more than $2 billion by the end of the decade. Major technical trends include higher power for sealed carbon dioxide (CO^sub^2) laser systems and Nd:YAG laser systems, new crystals for tunable solid-state laser systems, blue diode lasers with output near 623 nm, and 193 nm excimer lasers for industrial applications. These new laser technologies are replacing existing laser systems as well as other manufacturing techniques. Examples include using tunable solidstate lasers instead of dye lasers and diode lasers in place of helium neon (HeNe) laser systems. In related news, the US Department of Energys Argonne National Laboratory (Argonne, IL), operated by the University of Chicago, and Spawr Industries (Lake Havasu City, AZ), a specialist in high-power laser technology, are partners in a twoyear project to improve lenses and mirrors to shape laser beams for industrial use. The cooperative RD agreement also may improve laser thermal simulation in testing components found in jet engines and particle accelerators for their ability to withstand heat. The projects primary focus is improving Spawrs radiometric power meter for high-energy lasers. The conventional measurement method takes 10 sec to absorb the total laser beam; the Spawr instrument measures laser power in one second by sampling a known fraction of the laser light and has a wider application range. Additional research areas include improving coatings for Spawrs laser dump to handle high-power density laser beams. The dump absorbs laser energy while a beam is tuned for a specific application. An example of Nd:YAG lasers displacing conventional processing comes from the Raycon Laser Group (Ann Arbor, MI), developers of a flexible system for laser scribing and drilling powder-metal (PM) connecting rods. Two stress risers (the starting points for fracture splitting the lower crankshaft journal bore from the upper journal bore) are created by producing V-shaped notches 180 degreesapart in the crank bore. The rods-consisting of carbon copper, manganese, phosphorus, sulfur, silicon, chromium, nickel, oxygen, and iron-offer greater precision performance, improved weight accuracy over conventional metal rods, and reduced manufacturing costs. Prior scribing methods-broaching the Vshaped notch or producing it with a forging die-required higher forces to split the rods. The laser scribing process begins at the bottom of the journal surface and moves upward to blow slag through the kerf. The inside journal surface is mounted at 45 degreesto the beam to allow for nozzle clearance. Kerf width is between 0.008 and 0.010″ (0.20 and 0.25 mm), and depth is 0.020-0.030″ (0.51-0.76 mm). Feed rate, which affects kerf depth and the slag buildup, is 60 ipm (1524 mm/ min). To help produce a clean scribe, oxygen assistance at 70 psi (480 kPa) and a high-power YAG at 120 W also are used. According to Raycon, the force required to fracture a laser scribe stress riser is only 3000-8000 lb (13 500-35 500 N), whereas the force required for a broach scribe is 18,000 lb (80 000 N). The V-shape method is economical because it reduces the stages of manufacturing, increases the load-bearing capability of the part, and produces a perfect fit between the rod and cap.

The physical education curriculum includes, but is not limited to, basic locomotor patterns and skills, object control skills, rhythms and dance, aquatics, physical and motor fitness, and games. The unique and specific needs of individuals with disabilities may require accommodations and adaptive techniques or strategies to provide effective instruction in developmentally appropriate skills, functional skills, and age appropriate skills within the least restrictive environment. Instruction in Adapted Physical Education provides the foundation for learning and performing psychomotor skills, so that individuals with disabilities can more fully, safely, and with satisfaction participate in movement programs, recreation, sport, and physical fitness activities. The Adapted Physical Education Specialist teaches in all domains of learning with an emphasis on the psychomotor area. Instruction is provided in communication, cognition, social, behavioral, perceptual, and daily living domains, in addition to physical and motor skills. Student preferences and interests are considered in program planning, as well as coordination of services and resources.

HUDDERSFIELD Giants will be competing in a new competition this season. The Senior Academy has been scrapped and replaced by a Reserve Division. The major difference is that up to eight Under 21s are allowed in a Reserve line-up, while it was almost exclusively for Under 21s in the old Senior Academy. However, the Junior Academy for Under 18s remains, although it has been re-named the Academy. Both sides will play home games at the DRAM Centre at Moldgreen, although the Reserves play opening fixtures at YMCA.
